Your first visit to Madrona Dermatology will require some preparation and a little more time than your subsequent follow-up visits. Please arrive 20 minutes early. To understand what to expect and prepare for your visit, please read below:
Before your Appointment
Please print and read (and sign if indicated) the following forms:
- Welcome to Madrona Dermatology (complete this form)
- Notice of Privacy Practices (read-only form)
- Financial Policy (read/sign form)
- Credit Card on File (read/sign form)
- Patient Agreement (read/sign form)
- History and Intake (complete this form)

Day of Your Appointment
Please check in 20 minutes before your scheduled appointment time.
All patients under 18 years of age must be accompanied by a parent or legal guardian at their first visit.
In order to better serve you, please bring the following to your appointment:
- Photo ID
- Current insurance card
- Preferred credit card to use for your credit card on file
- Financial Policy form
- Credit Card on File form
- Patient Agreement form
- History and Intake form
- List of medications
- Pharmacy information
- Referring physician information
- Referral (if required by your insurance)
Please call 24 hours in advance of your appointment if you need to cancel or reschedule your visit.
Patient Portal
The Patent Portal is an online health record and communication tool available to all of our patients. It provides a secure communication channel between our practice and patients, which allows you to participate in the management of your health care. You can review your electronic health record (EHR), update demographic information, view lab results, request medication refills, and/or simply ask questions. If you would like to be registered, please let us know during your visit or contact us by phone/email.
