What to expect during your follow-up visits?
Please see below:
In order to better serve you, please bring the following to your follow-up appointments:
- Photo ID
- Current insurance card
- Preferred credit card to use for your credit card on file
- List of any changes in your medical history
- List of any medications changes
- Pharmacy information (if changed)
- Referral (if required by your insurance)
If you do not show up for your scheduled appointment or you cancel/reschedule your appointment without 24 hours notice, there will be a $50 fee. If you miss three appointments, you may be moved to walk-in status.
Patient Portal
The Patent Portal is an online health record and communication tool available to all of our patients. It provides a secure communication channel between our practice and patients, which allows you to participate in the management of your health care. You can review your electronic health record (EHR), update demographic information, view lab results, request medication refills, and/or simply ask questions. If you would like to be registered, please let us know during your visit or contact us by phone/email.
